Arrivals

• Please arrive 15 minute prior to your first appointment with us.
• Repeat clients, please arrive 5 minutes prior to your appointment.

Cancellations

• We respectfully ask that cancellations be made at least 24 hours prior to your scheduled appointment.
• We charge $50 for cancellations made within 24 hours of your appointment and for un-notified missed appointments. This will be charged to your credit card on file.
• Appointment no-shows may result in full payment of your session charged to your credit card.
• A charge for late cancellation or no-show may be voided if the appointment time slot is filled by another client.

Payments

• Payments are due in full by the end of each session.
• Gift certificates are to be presented prior to any session.
• Payments are accepted in cash, check or credit card.

Misconduct

• Please refrain form using profanity, speaking loudly or making obscene gestures. You will only have one warning and be asked to leave the premises after that.
• Any and all sexual contact, innuendo, intention to solicit or misconduct from either practitioner or client is purely unacceptable and forbidden.
• We reserve the right to terminate a session immediately.
• Any scheduled sessions in the future will be canceled without refund.

Client/Practitioner Expectations

• During your session please let us know if we can do anything to improve your experience.
• Upon completion of each session a customer survey is available which is helpful for us to improve and create an exceptional experience for your next session with us.
• Please use the suggestion box or online survey if there is anything we can do to improve our service for you.
• If for any reason you are dissatisfied with your service please fill out the survey form and speak with a manager on site.

Privacy

• Any basic client information such as name, phone number, address, email, appointments, etc. are available only to the client and any persons the client wishes to authorize in writing.
• In the case of joint memberships client information and appointments are kept separately and available only to each individual unless stated otherwise.
• Any and all client intake information is confidential and will be disclosed only if the client is either:

(a) A minor, in which case this information is available only to a parent or legal guardian.

(b) Yourself, the client giving consent via signed release form and to whom the information is disclosed.

This Privacy Statement/Policy applies to 345 Grand Avenue, Del Norte, Colorado 81132 and San Luis Valley Therapeutic Massage LLC ("SLV Therapeutic Massage") and governs data collection and usage. For the purposes of this Privacy Statement/Policy, unless otherwise noted, all references to the SLV Therapeutic Massage website are as an e-commerce website. By using the SLV Therapeutic Massage website ("the Site"), you consent to the data practices described in this Privacy Statement/Policy.

Children Under Eighteen
