Cancellation Policy:

· First appointment arrive 15 minute prior to Appointment

· Repeat clients please arrive 5 minutes prior to appointment

· We respectfully ask that cancellations be made up to 24 hours prior to scheduled appointment. We charge $50.00 for any cancellations within 24 hours of appointment or for unnotified missed appointments.  This will be charged to your credit card on file

· Any no shows may result in full payment of your session charged to your credit card.

Charges toward your credit card for late cancellation or no show may be void if the appointment time slot is filled by another customer.

Payment Policy:

· All payments are due in full by the end of each session

· Gift certificates are to be presented prior to any session.

· All payments are made Cash, check, credit card and Gold and silver (market value).

Misconduct:

· Please refrain form using profanity, speaking loudly or making obscene gestures. You will only have one warning and asked to leave the premises after that.

· Any and all sexual contact, innuendo, intention to solicit, or misconduct from either practitioner or client is purely unacceptable and forbidden. We reserve the right to terminate the session immediately. Any scheduled sessions in the future will be canceled without refund.

Client Practitioner Expectations:

· During the session, please let us know if we can do anything to improve your experience.

· Upon completion of each session a customer survey is available and is helpful for us to improve and create an exceptional experience for you for the next time you book your body work session with us.

·       Please use the suggestion box or online survey if there is anything we can do to improve our service for you.

·       If for any reason you are dissatisfied with your service fill out the survey form and speak with a manager onsite.

Privacy Policy:

· Any basic information such as name, phone number, address, email, appointments, etc. are only available to the client and any persons the client wishes to authorize in writing.

· Information for Joint membership and appointments are kept separately and only accessible by each individual unless stated otherwise.

· Any and ALL client intake information is confidential and will only be disclosed if the client is

a.) a minor. This information is accessible for the parent or legal guardian only.

b.) You, the client give consent via signed release form and to whom it is disclosed to.
